What does this card cover?
This is a discount card, not insurance. It is already activated for prescription and vision care discounts. An additional purchase is necessary to activate dental coverage.

How do I use the Pharmacy Discount Savings Plan?
Simply provide this card each time you go to one of over 61,000 network pharmacies to purchase prescriptions and make sure the pharmacy submits the pharmacy claim on-line. Remember you must use one of the participating pharmacies. Your card is pre-activated and has no expiration date. If the pharmacist has trouble processing a claim, please direct them to call the Pharmacy Help Desk number printed on the front of your card.

How does the Pharmacy Discount Savings Plan work?
Please note that not every prescription claim will receive a discount. This card is a “cash discount card” where card members pay at the point of purchase. Card members pay the lower of: 1) the discount card price, or 2) the pharmacy regular retail price. This plan is not insurance where card members pay a flat dollar co-pay, for example: $10.00. Members on average save about 20% off the retail price. Savings are usually higher on generics. This depends on what medications the card member is purchasing or depends on the markup of the pharmacy’s regular price. Some prescriptions have been known to have savings of up to 50-60% off regular retail price! Pricing for network pharmacies in your area are available in the members area of our website.

Where can I use the Pharmacy Discount Savings Plan?
You must use one of the 61,000 participating network pharmacies. Some of the larger pharmacies are listed on the back of your card. A complete listing and pharmacy look-up is available at the member’s are on our website at www.ezwbd.com. You may also call member services.

How do I use the Prescription Mail Order Plan?
Simply go to our website and click on the mail order link or call 800-677-4323. Mail order prescriptions are available for 30 and 90 day supplies and there is no shipping charge. It is convenient, safe and economical!

How does the Discount Vision Plan work?
Present your card at one of our network providers. Our network is nationwide and includes local optometrists and national chains. There is a small discount on eye exams, but huge savings on the cost of eyewear. A schedule of benefits is available on our website. Who can use the card?
This card covers the entire family.

How do I enroll?
This card is ready to use. There is no need to enroll for the prescription and vision card. Simply put your name and a nine-digit identification number (does NOT need to be a Social Security number) on the card prior to handing to the pharmacist.

How does the Discount Dental Plan work?
The discount dental is available for a nominal fee. We have several discount dental plans available from which you can choose. They start at a very low cost of only $79.99. In addition, the coupon coded listed on the front of your card entitles you to an additional 20% savings off the cost of any plan you choose!
